Thailand - Non-Immigrant (O) E-visa (close relatives of Thai nationals) - Service: Visa Requirements

Download, fully complete, print, and sign one copy of the Thailand visa application form. 

PLEASE NOTE: the application form is attached as a sample as well.

Please provide one clear copy of your current, valid passport's information page. 

You must provide a passport photo. This passport photo must match the requirements of a standard US passport photo. They must be taken at an official passport photo provider (pharmacy, Costco, post office).  You can upload a digitial photo here for us to process.

Photo Requirements:

- Your head must face the camera directly with full face in view.

- You must have a neutral facial expression or a natural smile, with both eyes open.

- Taken in clothing normally worn on a daily basis

- Taken in the last 6 months

- Use a plain white or off-white background

Your picture holding the photo and information page of the passport.

An original signed copy of the Thailand Declaration Form.

Copy of driver's license or state issued ID, or ORIGINAL major utility bill (Water, Gas, Electric, Sewage) for the most recent month, showing the applicant's name and current address. The address must not contain a PO Box. Address MUST match home address in your applicant profile.

Proof of adequate funds for the duration of applicant’s stay in Thailand

A copy of hotel reservations or other lodgings for the duration of the trip. The applicant's name must be on the reservation.

Proof of relationship of Principal Applicant to all other members of the Family Group (a child or step-child under the age of 26 years and/or any other relative who is, by reason of age or any infirmity of body or mind, wholly dependent on that person for his/her subsistence.)

A copy of immigration status in the United States such as a resident card, work, or study permit. 

The letter from your spouse indicated that you both are still married and he/she acknowledges of your visit.

Personal details of a family in Thailand, e.g. ID card, Passport and the visa page or stay permit in Thailand.

Enclose the documents from Section 1 (Gather Documents for your Thailand  Visa) and a print out of your order confirmation in an Overnight Envelope (Fedex, UPS, etc).
