DS-82 Eligibility Requirements
⚠️ You MUST meet ALL six eligibility requirements to use Form DS-82. If you don’t meet any one of these criteria, you must apply using Form DS-11 in person.
Your previous passport must be in your possession and you must submit it with your DS-82 application. It will be returned to you with your new passport.

Your most recent passport must have been issued when you were age 16 or older. Child passports (issued under age 16) require DS-11.

How to Fill Out Form DS-82 (Step-by-Step)
The DS-82 form must be completed online using Adobe Reader or printed and filled out by hand in BLACK INK. Do NOT sign the form until instructed to do so in Step 12.
Complete Instructions for Each Field
Enter your full legal name as it currently appears on your identification. Use commas to separate last, first, and middle names. If you have no middle name, leave that space blank.
Enter your date of birth in the specified format. Example: March 15, 1985 would be written as 03/15/1985.
Select M for male or F for female. This must correspond to your biological sex as it appears on your birth certificate or other identifying documents.
Enter the city/town and state (if born in U.S.) or city/town and country (if born outside U.S.). Examples: “Chicago, Illinois” or “Toronto, Canada”
Enter all 9 digits of your SSN with no dashes or spaces. By law, you must provide your SSN. If you’ve never been issued one, enter all zeros (000-00-0000) and attach a signed statement explaining why.
Providing your email allows the State Department to contact you about your application and send you status updates. Highly recommended.
