When to File DS-64
Lost Passport (Misplaced)
You cannot find your passport and you have searched thoroughly โ your home, office, vehicles, recent luggage, and any other places it could be. If you simply moved it and don't remember where, search before filing. Once invalidated, the passport is unusable forever.
Stolen Passport
Your passport was taken without your consent โ pickpocketed, burglarized, taken from luggage, or stolen during an assault. File DS-64 immediately and also file a police report. Stolen passports are at high risk of identity theft and trafficking, so prompt reporting protects you.
Damaged Beyond Use (Not DS-64)
If your passport is damaged but you still have it (water damage, torn pages, broken chip), DO NOT file DS-64. Instead, bring the damaged passport to a passport acceptance facility and apply for replacement using Form DS-11. The State Department will examine and cancel the damaged passport when you apply.
Mutilated or Significantly Altered (Also Not DS-64)
If your passport has been mutilated, altered, or has unauthorized stamps, treat it as โdamagedโ not โlost.โ Bring it with you to apply for DS-11 replacement.
How to File Form DS-64
You have three filing options: file online (fastest, recommended), file at your in-person passport replacement appointment, or mail the form.
Option 1: File Online (Recommended)
Go to travel.state.gov and use the online lost/stolen passport reporting tool. Provide your information and submit. The passport is invalidated immediately and you receive electronic confirmation. This is the fastest and most reliable filing method.
Option 2: File at DS-11 Appointment
Bring a completed but unsigned DS-64 to your in-person passport replacement appointment at the acceptance facility. The acceptance agent will witness your signature and submit DS-64 along with your DS-11 application. This is convenient if you're already going in person for replacement.

What Happens After You File DS-64
Your Passport is Permanently Invalid
Effective the moment you file DS-64 (online) or it is processed (mail), your passport is invalid worldwide. Any attempt to use it at a border, airport, or consulate will be flagged. The passport cannot be reactivated under any circumstances.
You Need DS-11 for Replacement
The replacement application must be filed in person using Form DS-11, not the mail-in DS-82. This is true even if your lost/stolen passport was issued recently. You will need:
- Completed but unsigned DS-11
- Evidence of U.S. citizenship (birth certificate or naturalization certificate โ not a copy of the lost passport)
- Government-issued photo ID
- One passport photo
