14. Can I use DS-11 to replace a lost passport?
Yes. If your passport was lost, stolen, or irreparably damaged, you must apply for a replacement using Form DS-11 in person โ€” you cannot use DS-82 renewal by mail. You also need to complete Form DS-64 (Statement Regarding Lost or Stolen Passport) which reports your passport as lost or stolen and prevents fraudulent use. Once you report a passport lost/stolen, it is permanently invalid even if found later.
15. What is the execution fee and who gets it?
The $35 execution fee is a separate charge paid to the passport acceptance facility (not the State Department) for verifying your identity, witnessing your signature, and processing your DS-11 application. The amount is $35 for everyone (adults and children). It is paid to the acceptance facility, with payment methods varying by location. DS-82 renewals have no execution fee (saving $35). If applying for both passport book and card on the same DS-11, you only pay the execution fee once.
16. Can I expedite my DS-11 after I've already submitted it?
Sometimes. If your application has not reached the processing center yet (first 1-2 days), contact the acceptance facility immediately โ€” they may be able to add expedited service if your packet has not been mailed. If your application is at the processing center, call the National Passport Information Center at 1-877-487-2778 to request adding expedited service for the additional $60 fee. For genuine life-or-death emergencies, the State Department can sometimes expedite significantly with documentation.
17. What if I make a mistake on my DS-11 form?
Before your appointment: for minor mistakes, draw a single line through the error, write the correction next to it, and initial the change; for major mistakes or multiple errors, complete a new DS-11 form (free to download). Do not use correction fluid or tape. During your appointment, the acceptance agent will review your form before you sign and help correct errors. After submission, minor clerical errors are often corrected by processors, but significant errors result in suspension and add 3-4 weeks to processing.
18. Do I need to include travel plans on DS-11?
The travel plan fields on DS-11 are optional, but providing this information is helpful. If you have specific travel plans, providing them helps processors prioritize your application. Approximate dates are fine โ€” you do not need an exact itinerary. If you do not have travel plans, you can leave these fields blank or write "Not yet determined." Travel plans do not affect whether your passport will be approved, how long it is valid, or your ability to travel anywhere. The exception: regional agency appointments require proof of travel within 14 days.
19. How do I track my DS-11 application status?
Track your DS-11 application online at passportstatus.state.gov. You will need your last name, date of birth, and last 4 digits of Social Security number. Status meanings: "Not Available" means the application has not reached the processing center yet (first 7-10 days); "In Process" means received and in the processing queue (the longest stage); "Approved" means the passport has been produced; "Shipped" means it has been mailed.
20. What documents will be returned to me?
You will receive your new passport first via USPS mail (1-2 weeks after approval, or 1-2 days with expedited delivery). Original citizenship documents (birth certificate, naturalization certificate, etc.) and name change documents return separately 2-4 weeks after the passport, sent in a separate envelope for security. Form DS-11, your passport photo, photocopies of ID, and damaged previous passports are not returned. Store returned citizenship documents in a safe place โ€” you will need them for future passport applications.
