You must provide 2 passport photos. These passport photos must match the requirements of a standard US passport photo. They must be taken at an official passport photo provider (pharmacy, Costco, post office) and should not be taken and printed via a home photo printer.
US Passport Photo Requirements:
- Your head must face the camera directly with full face in view.
- You must have a neutral facial expression or a natural smile, with both eyes open.
- Taken in clothing normally worn on a daily basis
- Taken in the last 6 months
- Use a plain white or off-white background
- Be sized correctly
- Printed in color on matte or glossy photo quality paper
- You cannot wear glasses.
- You cannot wear a hat or head covering.
- You cannot wear headphones or wireless hands-free devices.
Copy of computer generated round trip tickets or travel itinerary showing round trip travel entering Thailand and exiting Thailand. Preferably, the proof of departure documents should show your departure from the United States to Thailand and back. Additionally, the Applicant's name must be on the travel document.
Please provide one copy of your quarantine hotel reservation. This is required to obtain a visa and to quarantine upon arrival in Thailand. This reservation must coincide with your exact dates of travel. This must be uploaded to your order profile prior to sending us documentation and is required to complete the application on your behalf. Upload digital copy
A copy of your health insurance document with your name on it showing you are covered for all international medical expenses while in Thailand including COVID treatment and coverage of a minimum of 100,000 dollars. This must be uploaded to your order profile before you send us your application and is required to complete the visa form. This must also reflect your travel dates in Thailand or cover you for the entire travel period. In addition, you must provide a Foreign Insurance Certificate which must be completed, signed and stamped by the insurance company providing your policy.
