anyone whose:

issued when you were age 16 or over AND

- most recent passport is still
in your possession AND

- most recent passport was
issued more than 1 year ago and less than 15 years ago AND

- you need to change the name on
your passport.
* The above instructions will
take you to renew your current passport. Follow those
instructions and provide the appropriate name change documentation.

These instructions are suitable for anyone whose:

issued when you were age 16 or over AND

- most recent passport is still
in your possession AND

issued within one year from passport issuance date.
