Forms (DSP-11 & DSP-64)
The US
State Department requires that our customers complete the DS-11 application
online. In summary, you'll need to ensure you have Adobe
Acrobat Reader installed, complete an online form and then print out
the form.
Step 1.
Download Adobe Acrobat
Reader
Click
here to download Adobe Acrobat Reader free of charge. If you already
have Adobe Acrobat Reader and can print as specified, go to step 2.
Step 2.
Complete the DS-11: Application for Passport Online
Click
here to go to a secure US State Department site and fill out the
DS-11 using the Passport Application Wizard
Click "Continue" below Apply for a Passport Book. The
Passport Application System will then ask you a series of questions
regarding your passport application. Fill in where appropriate and then click "Create
Form" at the bottom. Create Form will launch Adobe Acrobat
Reader with your DS-11 pre-filled. Once you get to this, go to
step 3.
Step 3. Print the pre-filled DS-11
Print
the pre-filled DS-11 form. Forms printed blurred or illegible
will not be accepted. They must be printed in black print on
white paper. The paper must be 8 1/2 inches by 11 inches, with
no holes or perforations, at least medium (20 lb.) weight, and with
a matte surface. Thermal paper, dye-sublimation paper, special
inkjet paper, and other shiny papers are not acceptable.
The
DS-11 form will be presented and signed in person at an authorized
passport acceptance facility. DO NOT SIGN this application
prior to meeting with the agent.
Step 4.
Download DSP-64 Application Form

here to download the DSP-64: Statement Regarding Lost or Stolen Passport
The
DSP-64 form is used to cancel a lost or stolen passport. Fill out
as much as possible and present the form while you are at an authorized
passport acceptance facility.
