The US
State Department requires that both parents or
guardian appear when apply for a child. This statement of
consent is for a parent who is not able to appear before the
acceptance agent. Please complete the form and have it
witnessed and notarized. In summary, you'll need to ensure you have Adobe
Acrobat Reader installed, complete the form, print it out and have
it witnessed and notarized.
